Sailors from Navy Information Operations Command Georgia were disciplined following the collision of USS HARTFORD and USS NEW ORLEANS on 20 March 2009. Details are available in the 102 page JAG MANUAL Report. The story is here in Navy Times. This hurts.Among the JAGMAN Investigation recommendations:
- appropriate administrative action be taken for the Direct Support Element Officer based on his poor performance and inadequate supervision of his team.
- appropriate administrative action be taken for the ESM Supervisor based on his poor performance and inadequate supervision of his team.
- appropriate administrative action be taken for the ESM operator based on his poor performance.
To my knowledge, this is the first instance of DSE personnel being disciplined for their involvement in a casualty aboard ship.

I'm very curious as to how the CTs were involved with the sub's collision.

I think they just got caught up in the investigation - not the collision itself. I think they made some mods to radio that were not authorized. Believe that came out as part of the JAGMAN. Still waiting for a copy.
A link to the JAGMAN is at the bottom of the article. Reading the parts that aren't blacked out it looks like the CT's were more involved than you might think, namely ESM.

I do not know if any of the responders have been aboard an underway submarine. I have as COB, COW, and Diving Officer. We rarely received navigational orders from the Radio Room, if ever. The navigational team did, of course, receive notification of other ships in the area, probably from Radio. An investigation of a ships collision can bring out many things that are hidden from normal view. There was a time when the Captain went down with the ship but it looks like now they want to take half the ships complement as well.
